Please pay attention that your personal data does not apply to "archival" records. You should attach documents that confirm the proposed changes to your request. You cannot propose new information that is not documented in the records and any corrections that may be controversial in nature. Changes that may be requested are related to your personal information such as your name, social security number, date of birth or place of birth. If you are a retired veteran, amendments to your official records will be accepted only if the change happened during your tour of active duty or reserve service. Usually, this includes copies of documents that confirm the proposed changes. ![]() Your request must include additional information about the things you think are incorrect. Once you have decided to make corrections to your Certificate of Release or Discharge from Active Duty (DD Form 214), you have to submit your corrections on the DD Form 215 in written form. ![]()
